PA and NJ Licensed Telehealth Physician

Job Summary

Providing telehealth-based care, the full-time PA and NJ Licensed Telehealth Physician will manage patient outreach, conduct annual wellness visits, and support chronic disease management while working remotely with a preference for candidates residing in PA, NJ, or DE.

Key responsibilities

• Deliver patient-centered care and ensure an excellent patient experience
• Conduct annual screenings and wellness visits as assigned
• Collaborate with primary care providers to re-engage patients and connect them to necessary services

Required qualifications

• MD or DO degree, or equivalent
• Board certified or eligible in Family Medicine or Internal Medicine
• Minimum of 3 years of primary care experience
• Licensure in both PA and NJ required, or ability to obtain by start date
• Commitment to health equity and care for medically and socially vulnerable populations
