PA and NJ Licensed Telehealth Physician

Job Summary

To support patient access in a growing primary care network, a full-time PA and NJ Licensed Telehealth Physician will provide remote telehealth services, focusing on patient outreach, annual screenings, and chronic disease management while collaborating with primary care teams.

Key responsibilities

• Provide patient-centered care and ensure an excellent patient experience
• Conduct annual wellness visits and required screenings for patients
• Assist patients with chronic disease management and facilitate their reconnection to primary care providers

Required qualifications

• MD or DO degree, or equivalent
• Board certified or eligible in Family Medicine or Internal Medicine
• 3+ years of primary care experience
• Licensure in both PA and NJ required, or ability to obtain by start date
• Commitment to health equity and care for medically and socially vulnerable populations
