Job Summary
To support a dynamic healthcare team, the full-time remote PA or TN Licensed Clinical Pharmacist will provide clinical review and consultation on medication requests, evaluate therapy appropriateness, and collaborate with providers to ensure optimal patient care.
Key responsibilities
- Provide clinical review and consultation on medication requests, prior authorizations, and benefit determinations
- Evaluate therapy appropriateness, dosing, and safety based on evidence-based guidelines
- Identify opportunities to improve medication adherence, safety, and cost-effectiveness
Required qualifications
- R.Ph required; PharmD preferred
- Full, current, and unrestricted pharmacist license in PA or TN
- Ability to obtain additional state licensure based on business needs
- 1+ year of pharmacy experience preferred (retail, hospital, or managed care)
- Working knowledge of formulary management and clinical guidelines
