Job Summary
To support the Journey and Palliative program, the full-time Palliative Care Administrative Specialist will manage patient coordination, conduct follow-up calls, and serve as a resource for patients and their families while working remotely.
Key responsibilities
- Coordinate, schedule, and assign patients to providers within the Journey or Palliative programs
- Conduct follow-up calls to gather data on patient and family responses to treatment plans
- Serve as the primary contact and advocate for patients, families, and care teams, ensuring compliance with regulations and confidentiality
Required qualifications
- Minimum of 2 years of experience in Palliative, Hospice, or Home Care preferred
- Bachelor's degree in a health-related field preferred
- Experience with medical records and HCHB strongly preferred
- Proven data analysis skills with experience using electronic health records
- Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint
