Palliative Care Case Manager RN

Job Summary

To support patients with advanced Chronic Kidney Disease and End Stage Kidney Disease, the full-time remote Palliative Care Case Manager RN will manage complex Goals of Care discussions, coordinate care among healthcare providers, and empower patients to make informed healthcare decisions aligned with their values.

Key responsibilities

• Lead complex telephonic discussions to support patients in navigating treatment options and advance care planning
• Collaborate with healthcare teams to develop and update patient care plans, ensuring alignment across all providers
• Assess symptom burden and implement interventions to improve patients' quality of life while facilitating transitions to palliative care or hospice services

Required qualifications

• Minimum of 2+ years of experience as an RN in Palliative medicine required
• Minimum of five (5) years of experience as an RN required
• Current RN License is required, BSN preferred
• Current CPR or BLS certification
• Proficiency in EMR systems and virtual communication platforms
