Job Summary

A company is looking for a Clinical Associate - Palliative Care.

Key Responsibilities

• Coordinate palliative and hospice care orders, ensuring timely and accurate support for members
• Assist in triaging clinical and non-clinical issues, directing them to appropriate teams
• Provide patient education and advocate for members regarding care coordination issues

Required Qualifications, Training, and Education

• MA Certification is highly preferred, with consideration for significant MA experience
• Minimum of 2 years of outpatient experience in relevant healthcare settings
• Fully bilingual in English and Spanish
• Experience working with electronic health records and various computer platforms
• Ability to work flexible hours, including potential weekend coverage
