Job Summary
To support a growing virtual Palliative Care program, the full-time Palliative Care Nurse Practitioner will manage a panel of high-risk patients, perform assessments, and develop care plans while working remotely across multiple states.
Key responsibilities
- Conduct initial assessments and create Palliative Plans of Care for new patients enrolling in the program
- Provide ongoing telephonic and video consultations for a panel of approximately 40-50 patients, focusing on advanced symptom management and goals of care
- Collaborate with interprofessional teams and participate in regular meetings to ensure seamless care coordination and support during periods of growth
Required qualifications
- Current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state
- Minimum of 5 years in advanced practice, with at least 3 years in Palliative Care
- ACHPN credential or willingness to obtain within the first six months of employment
- Ability to work across multiple time zones and adapt to a fast-paced, technology-driven environment
- Experience in interprofessional teamwork and patient-centered care approaches
