Job Summary

A company is looking for a Hospice and Palliative RN.

Key Responsibilities

• Triage urgent palliative care needs and provide clinical support to regional palliative physicians
• Facilitate hospice referrals and maintain communication with hospice agencies to ensure continuity of care
• Conduct palliative-focused patient care tasks and assist with advance care planning documentation

Required Qualifications

• Graduate of an accredited School of Nursing
• Currently licensed as a Registered Nurse
• 2+ years of experience in palliative-focused care, geriatric home-based care, or hospice care
• Ability to travel to assigned markets up to 20%
• Familiarity with Microsoft Office and web-based applications
