Palliative Care RN

Job Summary

A company is looking for a Supportive Care Case Manager/Palliative Care RN - REMOTE.

Key Responsibilities

• Lead complex Goals of Care discussions and coordinate care plans for high-acuity patients
• Educate patients on palliative care options and facilitate transitions to appropriate services
• Assess symptom burden and implement nursing interventions to improve patients' quality of life

Required Qualifications

• Minimum of 2+ years experience as an RN in Palliative medicine required
• Minimum of five (5) years experience as an RN required
• 2+ years of care plan creation and proactive discharge planning experience
• Current RN License is required, BSN preferred
• Current CPR or BLS certification is required
