Palliative Care RN Case Manager

Job Summary

A company is looking for a Supportive Care Case Manager/Palliative Care RN to provide specialized support for patients with advanced Chronic Kidney Disease and End Stage Kidney Disease.

Key Responsibilities

• Lead complex Goals of Care discussions and advance care planning for high-acuity patients
• Coordinate care between various healthcare providers and facilitate transitions to palliative care or hospice services
• Educate patients on treatment options, advanced directives, and symptom management to improve quality of life

Required Qualifications

• Minimum of 2+ years experience as an RN in Palliative medicine
• Minimum of five (5) years experience as an RN
• Current RN License is required; BSN preferred
• Current CPR or BLS certification
• Proficiency in EMR systems and virtual communication platforms
