Palliative Care Specialist

Job Summary

To provide essential support to patients, the full-time Palliative Care Specialist will manage communication channels, develop referrals, and serve as a primary contact for patients and families in a fully remote setting.

Key responsibilities

• Develop and nurture referrals while ensuring accurate documentation
• Serve as a primary contact and advocate for patients and families, managing various communication channels
• Perform screenings and educational sessions over the phone to assess patient eligibility for levels of care

Required qualifications

• High School diploma/GED
• 2 years of Hospice and/or Home Health experience preferred
• Knowledge of medical terminology preferred
• Proficient in Microsoft Office suite, including Teams
• Detail-oriented with effective time management skills
