Palliative Care Specialist

Job Summary

To support a dynamic healthcare team, the full-time Palliative Care Specialist will manage patient referrals, serve as a primary contact for patients and families, and ensure compliance with healthcare regulations while working remotely.

Key responsibilities:

• Develop and nurture patient referrals while maintaining accurate documentation
• Serve as a liaison between patients, caregivers, and healthcare teams to facilitate access to care
• Perform screenings and educational sessions to assess patient eligibility for various levels of care

Required qualifications:

• High School diploma or GED
• 2 years of experience in Hospice and/or Home Health preferred
• Knowledge of medical terminology preferred
• Proficiency in Microsoft Office suite, including Teams
• Strong customer service skills and ability to manage concurrent demands
