Job Summary
A healthcare company is searching for a person to fill their position for a Partial Telecommute Clinical Care Coordinator.
Must be able to:
- Create a positive experience and relationship with the member
- Proactively engage the member to manage their care
- Conduct member assessments if needed
Qualifications for this position include:
- Must reside in King, Kitsap, or Snohomish County and be able to travel locally within the county up to 50% of the time
- Bachelor's Degree (or higher) in Social Work, Psychology, Sociology, Life Sciences or Nursing
- Must have resided within the local community for 1+ years
- Knowledge of culture and values and familiarity with resources available within in the local community
- Able to navigate a Windows PC and utilize Microsoft Office Word, Excel and Outlook
