Job Summary

A leading healthcare provider has an open position for a Partial Telecommute Uniform Assessment System Field Assessment Registered Nurse in the Brooklyn Area.

Must be able to:

  • Visit MLTC and Managed Medicaid members in need of long term care
  • Conduct face to face UAS assessments according to state guidelines, policies, procedures and protocols
  • Support members with medication reviews and connections to home and community base services

Required Skills:

  • Ability to conduct assessments in the home or at a Skilled Nursing Facility throughout Queens, Brooklyn, NY and surrounding Burroughs
  • Current, unrestricted RN license for the state of New York
  • 3 years clinical experience
  • Previous home care, hospice or field based case management experience
  • Computer and typing proficiency to enter and retrieve data in electronic clinical records
  • Experience with email, internet research, use of online calendars, and other software applications
