Job Summary

A home and community-based healthcare company needs applicants for an opening for a Partial Telecommuting Assessment Specialist in Coshocton County.

Core Responsibilities Include:

• Conducting program assessments
• Providing recommendations for eligibility
• Managing a caseload of Ohio Home Care consumers

Must meet the following requirements for consideration:

• Field based travel
• Active license as RN
• 12 months experience in home and community based services within the past 5 years
• Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs desirable
