Job Summary
A healthcare consulting and recruitment firm has a current position open for a Partial Telecommuting Field Health Assessment Registered Nurse Case Manager in Lansing.
Individual must be able to fulfill the following responsibilities:
- Coordinatef care assessing the member's condition
- Coordinate and monitor the care of Health Plan members and develop a nursing plan of care
- Participate in interdisciplinary conferences to review clinical assessments
Position Requirements Include:
- Traveling to member homes & provider facilities
- Area travel (mileage reimbursed)
- Valid, unrestricted state Registered Nurse (R.N.) license. BSN preferred.
- At least 2-3 years' experience in an acute care hospital
- Roughly 1-2 years' prior experience working in a field Case Management role
- Knowledge of Federal and State assistance programs (Medicare/Medicaid/etc)
