Job Summary

A healthcare company is filling a position for a Partial Telecommuting Utilization Review and Transition of Care Registered Nurse.

Individual must be able to fulfill the following responsibilities:

• Managing network participation, care with specialty networks, care with providers and transfers to alternative levels of care
• Identifying potentially unnecessary services and care delivery settings, and recommend alternatives if appropriate by analyzing clinical protocols
• Conducting admission review, post-discharge calls and discharge planning

Qualifications Include:

• Travel: Yes, 10 % of the Time - Valid drivers license and/or dependable transportation necessary (variable by region)
• Active RN license -Ability to be licensed in multiple states without restrictions
• Prior clinical experience preferably in an acute care, skilled or rehabilitation clinical setting
• Proficient with Microsoft Office including Word, Outlook and Excel
• Previous experience in utilization management, discharge planning and/or home health or rehab
