Job Summary

A company is looking for a Patient Care Advocate.

Key Responsibilities:
  • Close care gaps and improve member and provider experience through outreach and face-to-face interaction
  • Collaborate with providers to improve HEDIS measures and provide education on coding
  • Assist in resolving deficiencies impacting plan compliance and conduct telephonic outreach to members for preventive services
Required Qualifications:
  • Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related field
  • 2+ years of experience in direct patient care, social work, quality improvement, or health coaching
  • Preferred license/certification: LPN, LMSW, C-SW, LSW, or RN
