Job Summary

A company is looking for a Patient Care Coach to support patients in managing chronic conditions and improving therapy adherence.

Key Responsibilities

• Provide personalized coaching and education on device usage and therapy adherence
• Monitor patient compliance and usage data, coordinating follow-ups and troubleshooting as needed
• Document patient interactions and ensure compliance with regulatory and organizational standards

Required Qualifications, Training, and Education

• High school diploma or equivalent required; Associate's degree in a related field preferred
• One year of work-related experience in healthcare or customer service
• Experience in health care organizations or environments that bill insurance
• Knowledge of chronic conditions and associated therapies preferred
• Technical aptitude for device functionality and data analytics tools
