Job Summary

A company is looking for a Patient Care Coach.

Key Responsibilities

• Provide one-on-one mentoring and coaching to patients on device use and adherence
• Monitor patient adherence and usage data, providing timely feedback and support
• Coordinate with internal teams and external partners to address patient needs and ensure compliance

Required Qualifications, Training, and Education

• High school diploma or equivalent required; Associate's degree preferred
• One (1) year of work-related experience required; additional experience for senior and lead levels
• Experience in healthcare administrative roles, customer service, or related fields
• Familiarity with chronic conditions and durable medical equipment preferred
• Completion of required compliance training and continuing education programs
