Job Summary

A company is looking for a Patient Care Coach 8-4:30 MNT.

Key Responsibilities
  • Provide one-on-one mentoring and coaching to patients on device use and adherence
  • Monitor patient adherence and usage data, providing timely feedback and interventions
  • Document all patient interactions and coordinate with internal teams to resolve patient needs
Required Qualifications, Training, and Education
  • High school diploma or equivalent required; Associate's degree preferred
  • One year of work-related experience in healthcare or customer service
  • Experience in healthcare organizations that bill insurance or provide medical supplies
  • Familiarity with chronic conditions and associated therapies
  • Technical aptitude for understanding device functionality and compliance reporting tools
