Job Summary

A company is looking for a Patient Care Coach 8-4:30 MNT.

Key Responsibilities
  • Provide one-on-one mentoring and coaching to patients on device use and adherence
  • Monitor patient adherence and usage data, offering timely feedback and interventions
  • Coordinate with internal teams and external partners to resolve patient needs and ensure continuity of care
Required Qualifications, Training, and Education
  • High school diploma or equivalent required; Associate's degree preferred
  • One (1) year of work-related experience required; additional experience may be needed for senior and lead levels
  • Experience in healthcare administrative roles, customer service, or related fields
  • Foundational knowledge of chronic conditions and associated therapies preferred
  • Completion of required compliance training and continuing education programs
