Patient Care Coach

Job Summary

A company is looking for a Patient Care Coach - PART TIME 12:00 PM EST - 4:00 PM EST.

Key Responsibilities

• Provide one-on-one mentoring and coaching to patients on device use and adherence
• Monitor patient adherence and usage data, offering timely feedback and interventions
• Coordinate with internal teams and external partners to resolve patient needs and ensure continuity of care

Required Qualifications, Training, and Education

• High school diploma or equivalent required; Associate's degree in a related field preferred
• One year of work-related experience in healthcare or customer service required
• Experience in health care administration, billing, or insurance services preferred
• Familiarity with chronic conditions and associated therapies is a plus
• Completion of required compliance training and continuing education programs
