Job Summary

A company is looking for a Patient Care Coach.

Key Responsibilities
  • Provide one-on-one mentoring and coaching to patients on device use and adherence
  • Monitor patient adherence and usage data, providing timely feedback and interventions
  • Coordinate with internal teams and external partners to resolve patient needs and ensure continuity of care


Required Qualifications, Training, and Education
  • High school diploma or equivalent required; Associate's degree preferred
  • Related experience in healthcare administrative roles or customer service
  • One (1) year of work-related experience required; additional experience for senior and lead levels
  • Foundational knowledge of chronic conditions and associated therapies preferred
  • Familiarity with device compliance reporting tools and data analytics platforms
