Job Summary

A company is looking for a Patient Care Coach KP-8:00-4:30 CST.

Key Responsibilities
  • Provide one-on-one mentoring and coaching to patients on device use and adherence
  • Monitor patient adherence and usage data, offering feedback and interventions as needed
  • Coordinate with internal teams and external partners to resolve patient needs and ensure continuity of care
Required Qualifications, Training, and Education
  • High school diploma or equivalent required; Associate's degree preferred
  • One year of work-related experience in healthcare or customer service
  • Experience in a healthcare organization, pharmacy, or related field preferred
  • Familiarity with chronic conditions and related therapies and devices
  • Completion of required compliance training and continuing education programs
