Job Summary

A company is looking for a Patient Care Coach.

Key Responsibilities

• Provide one-on-one mentoring and coaching to patients on device use and adherence
• Monitor patient adherence and usage data, providing timely feedback and interventions
• Coordinate with internal teams and external partners to resolve patient needs and ensure continuity of care

Required Qualifications, Training, and Education

• High school diploma or equivalent required; Associate's degree preferred
• One year of work-related experience in healthcare or related fields required
• Experience in health care administrative, financial, or insurance customer services preferred
• Familiarity with chronic conditions and Durable Medical Equipment (DME) is preferred
• Completion of required compliance training and continuing education programs
