Patient Care Coach

Job Summary

A company is looking for a Patient Care Coach to support patients in managing chronic conditions through education and device utilization.

Key Responsibilities

• Provide one-on-one mentoring and coaching to patients on device use and adherence
• Monitor patient adherence and usage data, providing timely feedback and interventions
• Coordinate with internal teams and external partners to resolve patient needs and ensure compliance

Required Qualifications, Training, and Education

• High school diploma or equivalent required; Associate's degree preferred
• One year of work-related experience required; additional experience for senior and lead levels
• Experience in healthcare administrative roles or related fields preferred
• Foundational knowledge of chronic conditions and associated therapies
• Familiarity with device compliance reporting tools and remote monitoring technologies
