Patient Care Coordinator

Job Summary

A company is looking for a Patient Care Coordinator.

Key Responsibilities

• Serve as the first point of contact for potential patients, providing guidance about the program
• Manage patient caseloads and coordinate all aspects of patient travel and care
• Advocate for patients by addressing their needs and facilitating referrals and paperwork

Required Qualifications

• High School diploma or equivalent; a four-year degree in a healthcare-related field is preferred
• A minimum of 2 years' experience in patient engagement or care coordination roles
• Experience as a medical assistant, clinician, or care coordinator is a plus
• Proficiency with telephonic and virtual engagement platforms and care management software
• Knowledge of HIPAA and health plan regulatory requirements
