Patient Care Coordinator
Job Summary
A company is looking for a Patient Care Coordinator, National Remote.
Key Responsibilities
- Manage requests for services from members and providers, coordinating care needs
- Follow up on referrals and prior authorizations, providing administrative support to clinical staff
- Assist with discharge planning and maintain confidentiality of member information
Required Qualifications
- High School Diploma/GED or equivalent experience
- Must be 18 years of age or older
- 2+ years of work experience in a medical office or medical field
- Experience with computers and knowledge of medical terminology
- Ability to work full-time with flexibility for 8-hour shifts during business hours
