Patient Care Coordinator

Job Summary

A company is looking for a Patient Care Coordinator, National Remote.

Key Responsibilities

• Manage requests for services from members and providers, coordinating care needs
• Follow up on referrals and prior authorizations, providing administrative support to clinical staff
• Assist with discharge planning and maintain confidentiality of member information

Required Qualifications

• High School Diploma/GED or equivalent experience
• Must be 18 years of age or older
• 2+ years of work experience in a medical office or medical field
• Experience with computers and knowledge of medical terminology
• Ability to work full-time with flexibility for 8-hour shifts during business hours
