Patient Care Coordinator

Job Summary

A company is looking for a Patient Care Coordinator to support physicians, dietitians, and patients in a telemedicine environment.

Key Responsibilities

• Manage a panel of patients, serving as their primary point of contact for inquiries and issues
• Coordinate patient care by addressing general inquiries and routing clinical questions to the appropriate team
• Schedule and reschedule patient appointments while maintaining clinician availability and patient engagement

Required Qualifications

• Preferred Associate's or Bachelor's degree; Certified Medical Assistant (CMA) certification is strongly preferred
• At least two years of experience in a fast-paced medical practice
• Strong customer service drive and passion for helping patients
• Proficient computer skills, including familiarity with Slack and Google Workspace
• Motivation to pursue a career in clinical medicine is encouraged
