Job Summary

To support a rapidly expanding healthcare operation, the full-time Patient Care Coordinator will manage high-volume patient result calls, provide clinical and administrative support for in-home visits, and ensure timely communication of lab results while working remotely.

Key responsibilities
  • Conduct high-volume outbound patient result calls and post-visit appointment reviews within defined SLAs
  • Provide clinical and administrative support by validating labs and ensuring documentation readiness for in-home care delivery
  • Serve as a liaison between patients, providers, and internal teams to ensure continuity of care and manage clinical documentation
Required qualifications
  • At least 1 year of experience in a clinical healthcare environment, such as a Patient Care Coordinator or similar role
  • Working knowledge of HIPAA, patient privacy standards, and medical terminology
  • Experience using electronic health record (EHR) systems and healthcare software
  • Exceptional organizational skills and attention to detail, particularly with PHI and clinical documentation
  • Ability to adapt to changing priorities and work effectively in a fast-paced environment
