Job Summary
To support care management operations, the full-time Patient Care Coordinator will manage service requests, follow up on referrals and authorizations, and assist clinical staff while working remotely from anywhere within the U.S.
Key responsibilities:
- Manage requests for services from members and providers, coordinating care needs with clinical staff
- Follow up on incoming and outgoing referrals and assist with discharge planning under the direction of the ACM nurse
- Provide administrative support, including the creation and maintenance of system reports and handling inquiries from members and providers
Required qualifications:
- High School Diploma / GED or equivalent work experience
- 2+ years of experience in the healthcare industry and with healthcare insurance
- 2+ years of clerical or administrative support experience
- Experience with Microsoft Office Suite and knowledge of medical terminology
- Ability to work full-time (40 hours/week) with flexibility for 8-hour shifts during business hours
