Patient Care Coordinator

Job Summary

To support a growing pediatric practice, the part-time Patient Care Coordinator will provide clinical and clerical support, manage patient communications, and ensure quality care in a remote setting with a schedule of 3x12s (7:00am - 7:00pm EST) Monday-Wednesday or Wednesday-Friday.

Key responsibilities

• Accept and manage incoming patient calls, performing protocol-based triage and documenting phone notes for providers
• Ensure pre-visit preparations are completed by reviewing schedules and obtaining necessary patient information from various sources
• Assist the care team in managing patient needs and completing necessary forms for provider review and signature

Required qualifications

• High-school level education or equivalency
• 2-4 years of healthcare experience in outpatient clinics, preferably in specialty/multispecialty settings
• Spanish fluency preferred
