Patient Care Coordinator

Job Summary

To support referral management operations, the full-time Patient Care Coordinator will manage internal and external referrals, process required documentation, and provide exceptional customer service in a remote environment.

Key responsibilities

• Managing daily referral reconciliation activities and ensuring compliance with turnaround times
• Answering incoming calls to assist with scheduling and referral inquiries
• Collaborating with providers and patients to resolve referral-related issues and maintain accurate documentation

Required qualifications

• High School Diploma or equivalent
• 1+ years of referral management experience
• 1+ years of medical insurance experience
• Knowledge of basic medical terminology
• Proficiency in Microsoft Office applications
