Patient Care Coordinator - Remote

Job Summary

To support a growing outpatient therapy practice, the temporary Patient Care Coordinator - Remote will provide centralized support for referral processing, scheduling, insurance verification, and patient communication while working from home for a 3-month assignment.

Key responsibilities:

• Manage patient referrals and schedule evaluations and follow-up visits accurately and timely
• Deliver positive patient experiences through outbound calls, appointment reminders, and barrier resolution
• Assist with insurance workflows, including benefits verification and communication of coverage details

Required qualifications:

• High school diploma or equivalent
• 1+ year of experience in a healthcare front office, patient access, scheduling, call center, or insurance verification role
• Strong phone-based communication skills with professional and empathetic engagement
• Proficient in basic computer systems and multitasking across multiple platforms
• Demonstrated ability to manage time effectively and work independently in a remote environment
