Job Summary

A company is looking for an Existing Patient Care Navigator.

Key Responsibilities
  • Ensure documentation, authorization, and payor requirements for continued treatment are met
  • Communicate proactively with patients and internal teams to support continuity of care
  • Monitor ongoing patient needs and address issues to prevent delays or disruptions in care
Required Qualifications
  • Experience with electronic health records (EHRs) and customer relationship management (CRM) tools
  • Ability to work effectively across multiple systems and learn new platforms as needed
  • Experience in managing patient care workflows, including authorizations and scheduling changes
  • Proven ability to manage high-volume interactions and maintain quality of work
  • Demonstrated ownership and follow-through on tasks and issues
