Patient Care Navigator

Job Summary

A company is looking for a Patient Care Navigator-Care Management.

Key Responsibilities

• Manage patient caseloads through various communication methods and conduct outreach to encourage program participation
• Assist patients with self-care needs, including scheduling provider visits and accessing community resources
• Document activities and collaborate with the multidisciplinary clinical care team to evaluate program processes and recommend improvements

Required Qualifications, Training, and Education

• High school diploma or equivalent required; Bachelor's degree preferred
• MA, LPN, BSW, or Clinical Dietitian with appropriate accreditation/licensure preferred
• Experience working with patients who have chronic and behavioral health needs
• Demonstrated experience as part of a multidisciplinary clinical team
