Patient Care Navigator

Job Summary

Offering a hybrid schedule, the full-time Patient Care Navigator will manage patient caseloads through in-person, telephonic, and electronic communications, while conducting outreach and supporting self-care needs in a clinical setting.

Key responsibilities

• Conduct outreach calls to encourage patient participation in care management programs
• Assist patients with scheduling, transportation, and preparing for provider visits
• Collaborate with the multidisciplinary clinical care team to document activities and evaluate program processes

Required qualifications

• High school diploma or equivalent required; Bachelor's degree preferred
• MA, LPN, BSW, or Clinical Dietitian with appropriate accreditation/licensure preferred
• Experience working with patients who have chronic and behavioral health needs
• Demonstrated experience as part of a multidisciplinary clinical team
• Kidney Care experience strongly preferred
