• Provide empathetic support and timely responses to patient inquiries via phone, email, and messaging
• Assist with scheduling, telehealth services, and ensure accurate information relay between patients and providers

• 1+ years of experience in patient coordination, care management, or care navigation
• Strong technical skills and comfort with multiple systems and sensitive data
• A quiet, private workspace with reliable internet for confidential communications
• Ability to dedicate 30 hours per week to the role
