Key Responsibilities

• Improves patient experience and care continuity by interacting with patients and caregivers
• Assists in closing care gaps and communicates with patients and healthcare professionals
• Engages patients to address medication adherence issues and documents patient encounters

Required Qualifications, Training, and Education

• Bachelor's degree in a health-related profession or equivalent experience
• 1-3 years relevant experience in healthcare, patient navigation, or a related field
• Fluency in English; Spanish fluency is a plus
• Familiarity with electronic medical record and scheduling systems
• Intermediate proficiency in MS Office Suite
