Patient Engagement Coordinator

Job Summary

Owning the patient conversion funnel, the full-time Patient Engagement Coordinator will guide patients from referral to intake, ensuring timely access to care while managing outreach and re-engagement efforts in a remote capacity.

Key responsibilities

• Ensure timely contact with patient referrals and manage the conversion process to intake
• Conduct omnichannel outreach to educate and support new patients, addressing their questions and concerns
• Re-engage patients who miss appointments or drop out of care, helping them return to the program

Required qualifications

• Experience in customer service, call center, sales development, or healthcare coordination
• Ability to meet conversion and pipeline targets
• Strong communication skills to build trust and educate patients
• Organizational skills to manage multiple patient pipelines effectively
• Familiarity with CRM/EHR systems for accurate documentation
