Job Summary

A company is looking for a Patient Coordinator - Pilot.

Key Responsibilities:

• Call patients to confirm information, educate them on the program, and schedule onboarding
• Track patient progress in EMR/CRM systems and maintain detailed documentation for every patient interaction
• Support pilot program operations and coordinate with internal teams to ensure device shipments and patient readiness

Qualifications:

• 1-2 years of experience in healthcare, patient services, or customer support
• Experience in chronic care management is highly preferred
• Tech-savvy with comfort using multiple digital tools and platforms
• Experience with rehab, orthopedics, or post-surgical patients is preferred
• Familiarity with remote patient monitoring or home therapy programs is preferred
