Patient Engagement Specialist

Job Summary

A company is looking for a Patient Engagement Specialist who will serve as the primary point of contact for engaging patients and connecting them to care.

Key Responsibilities

• Outreach to schedule patient visits for Annual Wellness Visits and ensure timely preventative care
• Manage patient scheduling, including cancellations and reschedules, in collaboration with Nurse Practitioners
• Build trust and provide support to the geriatric population through effective communication and relationship management

Required Qualifications

• 1-2 years of post-high school education or a degree from a two-year college
• 1-3 years of experience in standard operating systems (Windows, MS Word, MS Excel)
• 1-3 years of call center experience, preferably in a sales-related role
• Professionalism in appearance, communication, and demeanor
• Ability to work independently with limited supervision
