Patient Engagement Specialist

Job Summary

Engaging patients to schedule and coordinate in-home and virtual health assessments, the full-time Patient Engagement Specialist will optimize outreach efforts to improve population health management while working remotely.

Key responsibilities

• Completes telephonic outreach to patients to promote and schedule health assessments
• Maintains targeted call averages and key metrics to ensure program success
• Educates patients on the benefits of assessments and coordinates appointments and medical equipment

Required qualifications

• Associate's degree in a healthcare-related field, LPN, or equivalent advanced training
• 1+ years of strong telephonic outreach experience or relevant skills
• Preferred healthcare experience
• Ability to operate in a fast-paced and dynamic environment
• Strong organization and customer service skills
