Patient Engagement Specialist

Job Summary

To support the Heart Failure Program, the full-time Patient Engagement Specialist will manage patient outreach, enrollment, and coordination while ensuring high levels of patient engagement and satisfaction in a remote role based near Orlando, FL.

Key responsibilities:

• Oversee and manage a referral list of patients eligible for enrollment
• Conduct outreach to patients, providing information on individualized care and assisting with appointment scheduling
• Handle inbound and outbound calls professionally, ensuring timely follow-up and maintaining accurate patient records

Required qualifications:

• High school diploma or GED
• Minimum one year of experience in customer service or a healthcare call center
• Experience in a medical office setting or healthcare is a plus
• Bi-lingual fluency in English and Spanish is required
• Familiarity with electronic health records and Microsoft Office applications is strongly preferred
