Patient Engagement Specialist

Job Summary

To support the Heart Failure Program, the full-time Patient Engagement Specialist will manage patient outreach, enrollment, and coordination while ensuring a high level of patient engagement and satisfaction in a remote capacity for candidates residing near Orlando, FL.

Key responsibilities:

• Oversee and manage a referral list of eligible patients for enrollment
• Conduct timely outreach to patients and maintain appropriate documentation of interactions
• Assist patients with scheduling appointments and ensure updated patient demographics in medical records

Required qualifications:

• High school diploma or GED
• Minimum one year of experience in customer service or a healthcare call center
• Experience in a medical office setting is a plus
• Bi-lingual fluency in English and Spanish is required
• Familiarity with electronic health records and computer applications, such as Microsoft Word and Excel, is strongly preferred
