Patient Engagement Specialist

Job Summary

To support patient engagement initiatives, the full-time Patient Engagement Specialist will process new referrals, validate insurance eligibility, and educate patients about organizational programs while working remotely.

Key responsibilities

• Informs and promotes organizational programs to patients, assisting with enrollment and administrative tasks
• Completes virtual outreach to patients referred from partnering providers and processes patient information into relevant systems
• Coordinates patient enrollment, including eligibility determination and scheduling with clinical team members

Required qualifications

• High School Diploma or GED required; Associate or Bachelor's degree in a health-related field is a plus
• Two or more years of customer service, sales, or direct patient-facing experience
• Experience working with the senior population in health service settings is preferred
• Ability to thrive in a fully virtual environment
• Empathy and passion for delivering an exceptional patient experience
