Patient Engagement Specialist

Job Summary

Connecting with patients to enhance their care experience, the remote Patient Engagement Specialist will manage outreach, schedule appointments for Annual Wellness Visits, and collaborate with Nurse Practitioners to ensure timely preventative care while working Monday to Friday from 8 AM to 5 PM PST.

Key responsibilities

• Outreach to patients for scheduling home, telehealth, and in-clinic visits, ensuring timely preventative care
• Coordinate daily patient schedules for Nurse Practitioners, managing cancellations and reschedules
• Build trust with the geriatric population, providing support and answering questions to enhance patient comfort

Required qualifications

• 1-2 years of post-high school education or a degree from a two-year college
• 1-3 years of experience in standard operating systems (Windows, MS Word, MS Excel)
• 1-3 years of experience in a call center, preferably in a sales-related role
• Experience handling protected health information (PHI) with sensitivity
• Professionalism in communication and appearance, with the ability to work independently
