Patient Enrollment Specialist

Job Summary

A company is looking for a Patient Enrollment Specialist to advocate for patients considering in-home care services.

Key Responsibilities

• Initiate outreach to patients and caregivers to explain the benefits of home-based care services
• Guide patients through the enrollment process, including eligibility verification and paperwork completion
• Maintain accurate records of outreach efforts and patient enrollment status in EMR systems

Required Qualifications

• High school diploma or equivalent required; associate or bachelor's degree in a related field preferred
• 1-3 years of experience in healthcare outreach or patient education
• Prior work in home health, hospice, or managed care preferred
• Familiarity with EMR systems; Athena preferred
• Bilingual abilities (especially Spanish) are a plus
