Job Summary
A company is looking for a Patient Experience Coordinator (Remote).
Key Responsibilities
- Complete individualized patient care plans and perform care management and coordination using the electronic medical record system
- Maintain frequent contact with patients to provide support and manage compliance with care management programs
- Document all client communications accurately and keep the Team Supervisor informed of any pertinent issues
Required Qualifications
- Comfortable with speaking on the phone for extended periods
- Compassionate and empathetic towards patients, demonstrating exceptional customer service
- Ability to take accurate notes and multitask efficiently
- Proficient in using a computer and Microsoft Office programs
- Knowledge of basic healthcare terms and principles is helpful
