Job Summary

A company is looking for a Patient Experience Coordinator (Remote).

Key Responsibilities
  • Complete individualized patient care plans and perform care management and coordination using the electronic medical record system
  • Maintain frequent contact with patients to provide support and manage compliance with care management programs
  • Document all client communications accurately and keep the Team Supervisor informed of any pertinent issues
Required Qualifications
  • Comfortable with speaking on the phone for extended periods
  • Compassionate and empathetic towards patients, demonstrating exceptional customer service
  • Ability to take accurate notes and multitask efficiently
  • Proficient in using a computer and Microsoft Office programs
  • Knowledge of basic healthcare terms and principles is helpful
